ABSTRACT

Objective@#To report a case of orbital subperiosteal abscess in a diabetic elderly patient.@*Methods@#This is a case report.@*Results@#A 65-year-old diabetic female consulted for a two-month history of poor vision on both eyes and a palpable mass on the left brow area. The patient previously underwent functional endoscopic sinus surgery and was treated for postoperative systemic infection and several complications including sinusitis, mastoiditis, trigeminal neuralgia, bacterial meningitis, and otitis media. On presentation, best-corrected visual acuity was noted to be no light perception on the right eye and 20/400 on the left eye. She had a relative afferent pupillary defect on the right eye, ophthalmoplegia on both eyes, and inferior displacement and proptosis of the left globe. Magnetic resonance imaging showed subperiosteal abscess on the left superior orbital rim with intraorbital extension. The patient underwent surgery and the culture from the drained abscess yielded Pseudomonas aeruginosa and Enterobacter cloacae. Pathologic examination of the orbital bone sample revealed osteomyelitis.@*Conclusion@#Osteomyelitis of the orbital bone leading to subperiosteal abscess is a complex, multifactorial disease requiring rapid diagnosis and treatment. Complete resolution of the infection must be achieved to decrease the morbidity and mortality of the patient.

ABSTRACT

Objective:To explore the establishment of a surgical complication monitoring mode based on data on the medical record homepage, and analyze its impact on the trend of changes in surgical complication incidence.Methods:A monitoring mode of surgical complication was developed based on the " structure-process-results" framework by using surgical complication rates derived from performance appraisal for a tertiary general hospital in Guangzhou. The number of surgical complications and the number of discharged surgical patients was collected from the hospital from January 2019 to June 2022 through the home page collection system for performance appraisal of national tertiary public hospitals. Descriptive analysis was used to analyze the incidence of surgical complications, and Joinpoint regression was used to analyze the trend of changes in the incidence of surgical complications. Monthly percentage change ( MPC) and average monthly percentage change ( AMPC) were calculated. Results:Since the hospital began implementing the surgical complication monitoring mode in May 2021, the incidence of surgical complications had decreased from 2.55% in June 2021 to 0.82% in June 2022, with an MPC of -5.58% ( P=0.024), which was better than the changes from January 2019 to May 2021 ( MPC=0.18%, P=0.755). Conclusions:The surgical complication monitoring mode constructed by the hospital can effectively reduce the incidence of surgical complications, providing reference for optimizing hospital′s medical quality management process and decision-making mode.

Resumen Objetivo: Comparar tres abordajes quirúrgicos (abierto, laparoscópico y laparoscópico convertido) para el manejo de complicaciones posoperatorias en cirugía colorrectal electiva realizadas primariamente por vía laparoscópica. Materiales y Método: Este estudio de cohorte retrospectivo incluyó pacientes reoperados después de una cirugía colorrectal laparoscópica electiva, agrupándose según la vía de abordaje de reoperación: abierta (RVA), laparoscópica (RVL) y laparoscópica convertida (RVLC). Las variables estudiadas fueron: preoperatorias (edad, sexo, puntuación ASA, IMC, comorbilidades e historia quirúrgica); operatorias (causa de reoperación, latencia para reoperación, tiempo operatorio, cirugía realizada y causa de conversión); y posoperatorias (tránsito intestinal, días de hospitalización, días de UCI, complicaciones médicas, infección del sitio quirúrgico, evisceración, transfusión y mortalidad a los 30 días). Resultados: Sin diferencias significativas para las variables preoperatorias y operatorias. En cuanto a las variables posoperatorias, el grupo de reoperaciones por vía laparoscópica, tuvo menos días de hospitalización (p = 0,012), menos días de UCI (p = 0,001) y un tránsito intestinal más rápido para reaparición de gases, heces y retorno a dieta sólida (p = 0,008, p = 0,029, p = 0,030, respectivamente). No hubo diferencias significativas en la infección del sitio quirúrgico, la evisceración, las complicaciones médicas, la transfusión y la mortalidad. Discusión y Conclusión: Este estudio reveló una mejor evolución clínica posoperatoria en el grupo de reoperación laparoscópica, con menor estancia hospitalaria y en UCI, y reducción del íleo posoperatorio, sin aumento de la morbimortalidad. Por lo tanto, la reoperación laparoscópica en cirugía colorrectal podría ser el abordaje más adecuado en pacientes debidamente seleccionados.


Aim: To compare three approaches (laparoscopic, open, and conversion of laparoscopic approach) for the management of intra-abdominal surgical complications after elective laparoscopic colorectal surgery. Materials and Method: This was a retrospective cohort study including patients who required reoperation due to an intra-abdominal surgical complication after initial elective laparoscopic colorectal surgery. Patients were grouped according to the reoperation approach-laparoscopic reoperation, laparoscopic reoperation that required conversion to open surgery, and open reoperation. Pre-operative variables (age, gender, ASA score, BMI, comorbidities, and surgical history), operative variables (cause of reoperation, latency for reoperation, operative time, surgery performed, and cause of conversion), and post-operative variables (intestinal transit, hospital days, ICU days, medical complications, surgical site infection, evisceration, transfusion and 30-day mortality), were compared between groups. Results: There were no significant differences between groups among the pre-operative and operative variables. In terms of post-operative variables, the laparoscopic reoperation group, had fewer hospital days (p = 0.012), fewer ICU days (p = 0.001), and faster intestinal transit regarding gas, stool and return to solid diet (p = 0.008, p = 0.029 and p = 0.030, respectively). However, there were no significant differences in surgical site infection, evisceration, medical complications, transfusion, and mortality. Discussion and Conclusión: This study revealed better post-operative clinical course in the laparoscopic reoperation group, with shorter hospital and ICU stay, and reduced postoperative ileus, without increased morbidity or mortality. Laparoscopic reoperation for complications after elective laparoscopic colorectal surgery may therefore be the preferred approach.

RESUMEN Fundamento: La enfermedad degenerativa cervical produce una variedad de síntomas clínicos que pueden ser tratados de forma no quirúrgica, sin embargo en ocasiones la cirugía es necesaria. La técnica quirúrgica más empleada para tratar esta enfermedad es la discectomía con artrodesis. El tratamiento adecuado de la hernia discal cervical depende de una adecuada selección de la técnica quirúrgica y su correcta realización. Objetivo: Caracterizar el tratamiento quirúrgico de la hernia discal cervical en el Hospital Provincial General Camilo Cienfuegos de Sancti Spíritus en el período: enero de 2015 a diciembre de 2018. Metodología: Se realizó un estudio prospectivo en el Hospital Provincial General Camilo Cienfuegos de Sancti Spíritus de enero 2015 a diciembre 2018. La población de estudio: todos los pacientes que acudieron a la consulta externa de neurocirugía del hospital refiriendo cervicalgia o cervicobraquialgia; la muestra: 51 pacientes diagnosticados por resonancia magnética nuclear de hernia discal cervical y que se les realizó al menos una discectomía cervical. Resultados: De los pacientes el 62.8 % eran mujeres y el 33.3 % tenía una edad comprendida entre 51-60 años. El espacio intervertebral más afectado fue C5-C6 (43.7 %). La técnica quirúrgica que predominó fue la discectomía con colocación de injerto autólogo (79.7 %). La complicación más frecuente fue la expulsión del injerto (3.9 %). Conclusiones: El comportamiento de la hernia discal en cuanto al sexo, edad y nivel afectado fue similar al descrito en la bibliografía. La técnica quirúrgica más empleada fue la artrodesis con injerto autólogo de cresta ilíaca. Las complicaciones presentadas fueron infrecuentes.


ABSTRACT Background: Cervical degenerative disease produces a variety of clinical symptoms that can be treated by a non-surgical way, however sometimes surgery is necessary. The most widely used surgical technique to treat this disease is discectomy with arthrodesis. The proper treatment of cervical disc herniation depends on an adequate selection of the surgical technique and its correct performance. Objective: To characterize the surgical treatment of cervical disc herniation at Camilo Cienfuegos General Provincial Hospital in Sancti Spíritus in the period: January 2015 to December 2018. Methodology: A prospective study was carried out at Camilo Cienfuegos General Provincial Hospital in Sancti Spíritus from January 2015 to December 2018. The study population: all the patients who attended the hospital outpatient neurosurgery consultation referring cervicalgia or cervicobraquialgia; the sample: 51 patients diagnosed with cervical disc herniation and performed at least one cervical discectomy. Results: Of the patients, 62.8 % were women and 33.3 % were between 51-60 years old. The most affected intervertebral space was C5-C6 (43.7 %). The predominant surgical technique was discectomy with autologous graft placement (79.7 %). The most frequent complication was the expulsion of the graft (3.9 %). Conclusions: The behavior of the herniation disc concerning sex, age and affected level was similar to that described in the literature. The most widely used surgical technique was arthrodesis with an autologous iliac crest graft. The complications presented were infrequent.

ABSTRACT

The presence of bowel contents during colorectal surgery has been related to surgical site infections (SSI), anastomotic leakage (AL) and postoperative complications theologically. Mechanical bowel preparation (MBP) for elective colorectal surgery aims to reduce fecal materials and bacterial count with the objective to decrease SSI rate, including AL. Based on many observational data, meta-analysis and multicenter randomized control trials (RTC), non-MBP did not increase AL rates or SSI and other complications in colon and even rectal surgery. In 2011 Cochrane review, there is no significant benefit MBP compared with non-MBP in colon surgery and also no better benefit MBP compared with rectal enemas in rectal surgery. However, in surgeon's perspectives, MBP is still in widespread surgical practice, despite the discomfort caused in patients, and general targeting of the colon microflora with antibiotics continues to gain popularity despite the lack of understanding of the role of the microbiome in anastomotic healing. Recently, there are many evidence suggesting that MBP+oral antibiotics (OA) should be the growing gold standard for colorectal surgery. However, there are rare RCT studies and still no solid evidences in OA preparation, so further studies need results in both MBP and OA and only OA for colorectal surgery. Also, MBP studies in patients with having minimally invasive surgery (MIS; laparoscopic or robotics) colorectal surgery are still warranted. Further RCT on patients having elective left side colon and rectal surgery with primary anastomosis in whom sphincter saving surgery without MBP in these MIS and microbiome era.

Resumen INTRODUCCIÓN La cesárea es el procedimiento quirúrgico que se realiza con el fin de obtener el nacimiento de un feto, con el objetivo de disminuir la morbimortalidad materna y perinatal asociada con el momento del nacimiento cuando el parto vaginal podría traer consigo complicaciones. En los últimos 30 años la incidencia de la cesárea se ha incrementado significativamente en casi todos los países occidentales, con valores que superan el 15% al 35%, este procedimiento está asociado a varias complicaciones, entre las más frecuentes se encuentran: infecciones, hemorragia obstétrica, histerectomía periparto, necesidad de unidad de cuidados intensivos y acretismo placentario. OBJETIVO Determinar la prevalencia de complicaciones quirúrgicas en pacientes sometidas a cesárea en un Hospital de alta complejidad y explorar factores asociados. MATERIALES Y MÉTODOS Estudio de prevalencia con análisis secundario en el que se incluyeron historias clínicas de pacientes sometidas a cesárea de urgencias y programadas. La variable resultado fue la complicación reportada durante un periodo menor a 30 días. Se tomó el universo, se recolectaron variables sociodemográficas, biológicas y clínicas. Para determinar la prevalencia se tomó como numerador el total de complicaciones y como denominador el total de historias analizadas, para explorar los factores asociados se utilizó el Odds Ratio como medida de asociación con un intervalo de confianza al 95%. RESULTADOS En 899 historias clínicas analizadas se encontró una prevalencia de complicaciones del 5.22% (IC95%: 3.86-6.89), dentro de los factores asociados positivamente se encontró tener una gestación pretermino (OR: 2.05, IC95%: 1.01-4.02); tener al menos una comorbilidad (OR: 2.51, IC 95%: 1.17-5.98) y la prioridad clasificada como urgente (OR: 3.01, IC 95%: 1.07-11.65). CONCLUSIONES La prevalencia encontrada en esta institución es similar a la de la mayor parte de los estudios publicados. El ser un procedimiento de urgencia, la presencia de comorbilidades siendo la preeclampsia la principal y el embarazo pretermino fueron los factores asociados positivamente con complicaciones, las más frecuentes fueron el requerimiento de transfusión, hemorragia, hematoma, la dehiscencia de sutura y la infección de sitio operatorio.


SUMMARY INTRODUCTION Caesarean section is the surgical procedure that is performed in order to obtain the birth of a fetus, with the aim of reducing maternal and perinatal morbidity and mortality associated with the time of birth when vaginal delivery could lead to complications. Last 30 years the incidence of caesarean section has increased significantly in almost all western countries, with values exceeding 15% to 35%, this procedure is associated with several complications, among the most frequent are: infections, obstetric hemorrhage, peripartum hysterectomy, need for intensive care unit and placental acretism. OBJECTIVE To determine the prevalence of surgical complications in patients undergoing caesarean section in a hospital of high complexity and explore associated factors. MATERIALS AND METHODS Prevalence study with secondary analysis, which included clinical records of patients undergoing emergency caesarean section and scheduled. The outcome variable was the complication reported for a period of less than 30 days. The universe was taken, sociodemographic, biological and clinical variables were collected. To determine the prevalence, the total number of complications was taken as a numerator and as the denominator the total of analyzed stories, to explore the associated factors the Odds Ratio was used as a measure of association with a 95% confidence interval. RESULTS In 899 clinical histories analyzed a prevalence of complications of 5.22% was found (95% CI: 3.86-6.89), among the positively associated factors it was found to have a preterm pregnancy (OR: 2.05, 95% CI: 1.01-4.02); have at least one comorbidity (OR: 2.51, 95% CI: 1.17-5.98) and the priority classified as urgent (OR: 3.01, 95% CI: 1.07-11.65). CONCLUSIONS The prevalence found in this institution is similar to that of most of the published studies. Being an emergency procedure, the presence of comorbidities being the main preeclampsia and preterm pregnancy were the factors positively associated with complications, the most frequent were the requirement of transfusion, hemorrhage, hematoma, suture dehiscence, operative and the site infection.

ABSTRACT

Introduction Reoperations are a common scenario among glioma patients. There is crescent evidence of its benefit in low- and high-grade gliomas. Here we discuss our experience with inert expanded polytetrafluoroethylene (ePTFE) dura substitute in glioma surgeries. Technical note We generally put the ePTFE dura substitute below the dura of the patient, even if it is intact. This membrane should be sutured in place using a tensionfree technique, with 4-0 polypropylene. Expanded polytetrafluoroethylene minimizes tissue attachment and fibrosis when performing reoperation in glioma patients. Discussion Since the literature has shown benefits in survival with reoperation in glioma patients, the use of ePTFE dura substitute can improve surgical time and minimize complications in a second surgery.

ABSTRACT

An 84-year-old man electively underwent abdominal aortic open repair for an abdominal aortic aneurysm. During the operation, the ureter was not confirmed when manipulating the iliac arteries. Subsequently, intestinal paralysis occurred on the fifth day after surgery and a drainage tube of the intestinal tract was inserted. Liquid retention around the left iliopsoas muscle, and left renal nephropathy were recognized on performing enhanced computed tomography (CT) on postoperative day 11. An initial diagnosis of an iliopsoas abscess was considered. Simple CT imaging was performed on the 13th day after surgery without symptomatic improvement. This scan revealed that the contrast agent had remained in the cavity since the previous CT scan, which had been misdiagnosed as an abscess. Ureteral injury was now suspected. Retrograde ureterography revealed an urinoma caused by left ureter injury. We diagnosed paralytic ileus due to urinoma. For drainage of the urine, a percutaneous renal fistula was constructed. He was discharged from the hospital on the 56th postoperative day, and by six months after the operation, the urinary tract problem had disappeared. In conclusion, we report a case of delayed ureteral injury that occurred after abdominal aortic open repair surgery.

ABSTRACT

Objective To summarize the experience of complex hepatic artery reconstruction in orthotopic liver transplantation. Methods Clinical data of 7 liver transplantation recipients who underwent complex hepatic artery reconstruction from January 2015 to March 2019 were retrospectively analyzed. Among them, 4 recipients received classical liver transplantation and 3 cases underwent piggyback liver transplantation. Intraoperative general conditions including anhepatic phase, intraoperative blood loss, hepatic artery anastomosis time and operation time of the recipients were recorded. The clinical prognosis and complications were observed. Results In two donors, variant right hepatic artery was used for vascular reconstruction. The celiac trunk or the common hepatic artery of the donors was anastomosed with the common hepatic artery of the recipients. Iliac artery bypass was employed in 2 cases, and then the hepatic artery of the donors was anastomosed with the abdominal aorta of the recipients. The superior mesenteric artery of 1 donor was end-to-end anastomosed with the common hepatic artery of the recipient. The celiac trunk of 1 donor was anastomosed with the splenic artery of the recipient. Only 1 case was required to undergo secondary liver transplantation due to acute hepatic artery thrombosis after hepatic artery anastomosis. All the 6 recipients successfully completed the liver transplantation. No perioperative death was observed. The anhepatic phase endured from 49 to 77 min. The intraoperative blood loss was ranged from 300 to 1 500 mL. The anastomosis time of hepatic artery was 23-56 min. The operation time was ranged from 5.3 to 11.1 h. The length of postoperative hospital stay was 23-56 d. Neither hepatic artery thrombosis nor stenosis occurred. The liver function of all recipients was basically restored to normal within postoperative 2 weeks. No severe surgical complications occurred. The liver graft achieved excellent function. Conclusions Appropriate identification of the hepatic artery variation, proper management of liver artery of the donors and recipients and reconstructing the blood supply of liver graft are the crucial procedures of liver transplantation.

Objective To establish an adverse event monitoring system in the course of encoding medical record homepages, using the internationally accepted Clavien-Dindo surgical complications grading standard, for the purpose of hospital′s medical safety management. Methods The surgical complications record system was established based on encoded medical record homepages. Data of three types of medical therapies of the discharge cases of a tertiary hospital in 2017 were studied and the incidence, type and grading of the surgical complications were analyzed, to identify doctors′missed diagnoses. Results A total of 588 surgical complications were detected from the discharge cases throughout the year. The incidence of surgical complications was 1.1% (588/52 319). The highest proportions of all surgical complications were infection and surgical incision problems. Grade Ⅱ complications accounted for the highest incidence rate (41.5% , 244/588); the proportion of missed surgical complications at the initial homepage was 62.2% (366/588), most of which being as grade Ⅱ complications ( 42.1% , 154/366). Conclusions The hospital adverse event monitoring system established based on the encoded medical record homepages features unified standard, making it feasible and universal, which serves an important role of the medical adverse event reporting system.

ABSTRACT

Numerosas enfermedades requieren como tratamiento una cirugía, ya sea de emergencia o electiva. Las cirugías pueden tener diversas complicaciones; sin embargo no se consideran las posibles alteraciones cognitivas postoperatorias: delirio del despertar anestésico, delirio postoperatorio y disfunción cognitiva postoperatoria (DCPO). La DCPO es un síndrome clínico caracterizado por alteraciones en la memoria, concentración, atención, comprensión del lenguaje e integración social posterior al uso de anestesia y cirugía que persiste después de tres meses; aunque algunos reportes consideran que no sería una entidad clínica individual. Su incidencia varía del 5,1 al 52,5% dependiendo de la población estudiada, tipo de cirugía, tiempo de la evaluación desde la cirugía y batería neuropsicológica empleada. Los factores de riesgo para DCPO son: edad avanzada, deterioro cognitivo previo, gravedad de enfermedades coexistentes, severidad de la cirugía, duración y tipo de la anestesia, ocurrencia de complicaciones y nivel educativo; siendo imprescindible la realización de una evaluación neuropsicológica antes y después de la cirugía para su diagnóstico. Los mecanismos etiopatogénicos propuestos son: la edad, cambios en la perfusión cerebral, disturbios del sueño, inflamación, efectos de los agentes anestésicos, y fundamentalmente la neuroinflamación. La DCPO es una entidad frecuente y escasamente sospechada, aunque su diagnóstico es controversial. Se sugiere que los pacientes que serán sometidos a cirugías electivas mayores, en especial las cardiacas y cerebrales, sean sometidos a evaluación neuropsicológica previa y a los 3 meses, así como a monitoreo electroencefalográfico intraoperatorio para disminuir la incidencia de DCPO.


Many diseases require surgery as a treatment, either emergency or elective. Surgeries can have various complications; However, the possible postoperative cognitive alterations are not considered: delirium of anesthetic awakening, postoperative delirium and postoperative cognitive dysfunction (PCDP). DCPO is a clinical syndrome characterized by alterations in memory, concentration, attention, understanding of language and social integration after the use of anesthesia and surgery that persists after three months; although some reports consider that it would not be an individual clinical entity. Its incidence varies from 5.1 to 52.5% depending on the population studied, type of surgery, time of evaluation from surgery and neuropsychological battery used. The risk factors for DCPO are: advanced age, previous cognitive impairment, severity of coexisting diseases, severity of the surgery, duration and type of anesthesia, occurrence of complications and educational level; It is essential to carry out a neuropsychological evaluation before and after the surgery for its diagnosis. The proposed etiopathogenic mechanisms are: age, changes in cerebral perfusion, sleep disturbances, inflammation, effects of anesthetic agents, and fundamentally neuroinflammation. The DCPO is a frequent and rarely suspected entity, although its diagnosis is controversial. It is suggested that patients who will undergo major elective surgeries, especially cardiac and cerebral surgeries, undergo previous neuropsychological evaluation at 3 months, and intraoperative electroencephalographic monitoring to reduce the incidence of DCPO.

ABSTRACT

<p><b>Objective</b>To investigate the diagnosis and management of penile fracture.</p><p><b>METHODS</b>From June 1993 to May 2017, 46 cases of penile fracture were treated in our hospital, averaging 33.5 (25-42) years of age and 3.45 (1-10) hours in duration, of which 41 occurred during sexual intercourse, 4 during masturbation and 1 during prone sleeping, 4 with hematuria, but none with dysuria or urethral bleeding. Hematoma was confined to the penis. Emergency surgical repair was performed for all the patients, 45 under spinal anesthesia and 1 under local anesthesia, 16 by coronal proximal circular incision and the other 30 by local longitudinal incision according to the rupture location on ultrasonogram. The tunica albuginea ruptures averaged 1.31 (0.5-2.5) cm in length, which were sutured in the "8" pattern for 6 cases and with the 3-0 absorbable thread for 18 cases. The skin graft or negative pressure drainage tube was routinely placed, catheters indwelt, and gauze used for early pressure dressing. In the recent few years, elastic bandages were employed for 3-5 days of pressure dressing and antibiotics administered to prevent infection. The stitches and catheter were removed at 7 days after surgery.</p><p><b>RESULTS</b>Short-term postoperative foreskin edema occurred in 14 of the 16 cases of circular degloving incision, but no postoperative complications were observed in any of the cases of local incision. Twenty-eight of the patients completed a long-term follow-up of 49.4 (10-125) months, which revealed good erectile function, painless erection, and satisfactory sexual intercourse.</p><p><b>CONCLUSIONS</b>For most penile fractures, local longitudinal incision is sufficient for successful repair of the tunica albuginea, with mild injury, no influence on the blood supply or lymph reflux, and a low rate complications. It therefore is obviously advantageous over circular degloving incision except when the cavernous body of urethra is to be explored, which necessitates circular degloving incision below the coronal groove.</p>

ABSTRACT

Objective To study the treatment principles and surgical skills in laparoscopic subtotal cholecystectomy (LSC) for acute cholecystitis.Methods We retrospectively analyzed the clinical data of patients who underwent LSC for acute cholecystitis from Jan.2006 to Dec.2015 at the Beijing Shijitan Hospital,Capital Medical University.We dissected any serious pericholecystic adhesions according to the principle that "It is better that the gallbladder rather than other tissue is injured",and the technique that "After the gallbladder anterior wall is excised,the gallbladder ampulla and duct are split along the longitudinal direction of the cholecystic duct,then the opened cholecystic duct is sutured inside the gallbladder".Results LSC was completed successfully in 96 patients.There were no conversion to open surgery,and no bile duct injury.The mean surgery time was (108.0 ± 37.0) min,the mean blood loss was (121.0 ± 62.0) ml,the mean peritoneal drainage was (105.0 ± 32.0) ml.The drainage tube was removed at a mean of (3.4 ±1.2) d after surgery.The mean hospitalization time after surgery was (6.1 ± 2.2) d.Surgical complications occurred in 2 patients with bleeding after surgery.One patient underwent laparoscopic exploration to stop bleeding.Another patient underwent conservative treatment and the bleeding stopped spontaneously.There were 3 patients who had mild bile leakage.All these patients recovered well after drainage.No patient developed bile duct stenosis or obstructive jaundice on follow-up.Conclusions LSC for acute cholecystitis was safe.Bile duct injuries could be avoided if we follow the principle of "It is better that the gallbladder rather than other tissue is injured" and the technique of "After the gallbladder anterior wall is excised,the gallbladder ampulla and duct are split along the longitudinal direction of the gallbladder,then the opened cholecystic duct is sutured inside the gallbladder".

Resumen Objetivo: Evaluar las características clínicas y bioquímicas de las pacientes con insuficiencia ovárica primaria. Materiales y métodos: Estudio observacional, ambispectivo y analítico, efectuado en pacientes atendidas entre los años 2005 a 2017 en el Instituto Nacional de Perinatología Isidro Espinosa de los Reyes con diagnóstico de insuficiencia ovárica primaria. Las pacientes se clasificaron en grupos según el origen del padecimiento. Se utilizó la prueba de ANOVA con corrección de Bonferroni para identificar diferencias en las variables cuantitativas en los cuatro grupos de estudio. Se realizó un modelo no lineal de tipo CHAID (X2 automatic interaction detector), con la finalidad de predecir la edad al inicio de la terapia hormonal, según el origen de la insuficiencia ovárica. Resultados: Se estudiaron 98 pacientes; 70 de ellas (71%) se clasificaron idiopáticas; 16 (17%) cromosómicas y 12 (12%) autoinmunitarias órgano-específicas. En las características clínicas se observó que las pacientes con etiología cromosómica tuvieron menopausia espontánea antes de los 30 años (80%); en las de causa quirúrgica fue después de los 30 años (53.8%). En relación con la densidad mineral ósea las mujeres con insuficiencia ovárica primaria de origen cromosómico resultaron con menos masa ósea al momento del diagnóstico en comparación con las demás causas; la columna lumbar (L1 a L4) resultó la más afectada. Conclusión: La principal causa de la insuficiencia ovárica primaria espontánea en la muestra estudiada fue idiopática, seguida de la cromosómica y autoinmunitaria órgano-especifica. El 12% tenía antecedente familiar, en primer grado, de insuficiencia ovárica primaria. Es obvia la necesidad de la asesoría genética.


Abstract Objective: Evaluate the clinical and biochemical characteristics of patients with primary ovarian failure. Materials and Methods: An observational, cross-sectional and ambispective study carried out in patients treated between 2005 and 2017 in the National Institute of Perinatology with a diagnosis of primary ovarian failure. The patients were classified into groups according to the origin of the condition. The ANOVA test with Bonferroni correction was used to identify differences in the quantitative variables in the four study groups. A nonlinear model of CHAID type (X2 automatic interaction detector) was performed, with the purpose of predicting the age at the beginning of the hormonal therapy according to the origin of the ovarian insufficiency. Results: 98 patients were studied; 70 of them (71%) were classified idiopathic; 16 (17%) chromosomal and 12 (12%) autoimmune organ-specific. In the clinical characteristics, it was observed that patients with chromosomal etiology had spontaneous menopause before the age of 30 (80%); in those of surgical origin it was after 30 years (53.8%). In relation to bone mineral density, women with primary ovarian insufficiency of chromosomal origin were found to have less bone mass at the time of diagnosis compared to the other causes; the lumbar spine (L1 to L4) was the most affected. Conclusion: The main cause of spontaneous primary ovarian failure in the sample studied was idiopathic, followed by chromosomal and organ-specific autoimmune. 12% had a family history, in first grade, of primary ovarian failure. The need for genetic counseling is obvious.

ABSTRACT

Smartphones and electronic tablets (e-tablets) have become ubiquitous devices. Their ease of use, smartness, accessibility, mobility and connectivity create unique opportunities to improve quality of surgical care from prehabilitation to rehabilitation. Before surgery, digital applications (Apps), serious games and text messaging may help for a better control of risk factors (hypertension, overweight), for smoking cessation, and for optimizing adherence to preoperative recommendations (e.g., regarding anticoagulation or antihypertensive treatments). During surgery, Apps may help to rationalize fluid management and estimate blood loss. After surgery, smartphones and/or connected sensors (pulse oximeter, adhesive path, electronic tattoo, bioimpedance necklace) can be used to monitor body temperature, heart rate, heart rate variability (detection of cardiac arrhythmia), respiratory rate, arterial oxygen saturation and thoracic fluid content. Therefore, these tools have potential for the early detection of infectious, cardiac and respiratory complications in the wards and from home. When connected to echo probes, smartphones and e-tablets can also be used as ultrasound devices during central venous catheter insertion, for peripheral nerve blocks, and to perform echocardiography in patients developing cardiac complications. Finally, electronic checklists now exist as Apps to enhance communication between patients and healthcare professionals, and to track and record step by step each element of the surgical journey. Studies are now urgently needed to investigate whether this digital revolution can translate into a better outcome, an earlier detection of postoperative complications, a decrease in hospital readmissions and in health care costs.

ABSTRACT

Objective To analyze the complications,treatments and prognosis of simultaneous pancreas-kidney transplantation,especially on surgical complications and treatments.Method The causes and outcomes of surgical treatment in 70 cases of simultaneous pancreas-kidney transplantation performed between Dec.1999 and June 2012 were retrospectively analyzed in our center.Result Sixteen patients (22.9%) underwent one or more reoperations.The causes for reoperation were as follows:2 cases of hematuria,4 cases of abdominal hemorrhage,4 cases of abdominal infection,4cases of pancreatic thrombosis,2 cases of renal graft's artery rupture,1 case of renal allograft rupture,1 case of intestinal fistula,and 1 case of pancreatic fistula.Eight pancreas grafts were lost in the first year.Pancreatectomy was performed on the other 5 cases:4 cases of pancreatic thrombosis,1 case of intestinal fistula,accounting for 43.8% of the patients subject to reoperation.The recipients,kidney,pancreas survival rate in reoperation group at 1 year was 87.5%,75%,and 56.3% respectively; and that in control group at 1 year was 98.1%,98.1 %,and 98.1 % respectively.There was significant difference in kidney survival rate (P<0.01,chi-square =6.79),and pancreas survival rate (P<0.01,chi-square =17.47) between two groups.Conclusion Although simultaneous pancreas-kidney transplantation provides a successful and effective treatment for diabetics with end-stage renal disease,surgical treatment due to complications is still an important factor in short-term survival on the grafts.

ABSTRACT

Objective to observe the efficacy and application value of PFNA and Gamma in the treatment of intertrochanteric fractures in elderly. Methods 100 patients with intertrochanteric fractures were divided into PFNA group and Gamma group, who received PFNA treatment and Gamma treatment respectively. And the operation status of patients at different ages,postoperative recovery,and complications were observed. Results The operation time,intraoperatve blood soss of PFNA group were lower than those of Gamma group(P<0. 05). There was no significant difference in fracture healing time. The Harris score of the over 75s in PFNA group was higher than that in Gamma group(P<0. 05),and the complications were less than Gamma group(P<0. 05). Conclusion PFNA was suitable for the patients at differ-ent ages with the advantages of more rigid fixation,good anti-rotation,fewer trauma and fewer complications.

ABSTRACT

Objective: To investigate the outcome of secondary intraocular lens implantation, compare final visual outcome between different categories of surgeon, and evaluate care provided by teaching hospitals to patients with capsular complications. Materials and Methods: Setting: Teaching hospital. Design: Retrospective study. Subjects were recruited by examination of electronic medical records. All patients operated for corrective surgery following capsular complications during cataract surgery were included. All patient medical records were reviewed, and data were collected for 359 eyes. Main outcome measures: Visual acuity and major complications. All collected data were entered into Microsoft Excel and analyzed by SPSS 17 software using cross tabulation and Chi-squared tests. Results: Surgical intervention made a significant difference to the final visual outcome (P < 0.001). The category of the trainee had a significant effect on the final visual outcome (P = 0.021). Conclusion: Capsular complications during cataract surgery should be surgically treated to improve outcome.
